This document must include the following mandatory fields: - Company name - Prefect’s letterhead sheet - Check that the Receipt number is filled in. - Verify that the supplier is authorized to transport DANGEROUS AND NON-DANGEROUS waste. - Signature of the Prefect.
Duration of the document: 5 years. Enter "Document Date": the date of authorization. For example, if the authorization is issued on 07 June 2020, please put Document date: 07/06/2020. |

It is possible to file a certificate of honor if you are not concerned by the document by justifying with date, company name, company in mind and signature.
⛔️ Documents not accepted
Only the certificate or a sworn statement must be put, no other document is allowed.
