The document must include the following fields:
- Company name (Supplier)
- Signatory Name
- Company signature and/or stamp
- Both pages required
No period of validity. The date corresponds to the date of the signature of the document for example: if the attestation is signed is on December 10, 2019, you upgrade the document date: 10/12/2019.
